Tenant Case Management
Tenant Case Management provides individualized supports through assessment, planning, assistance, supportive counseling, mediation, linkage with community resources, coordination with mental health, developmental disabilities, substance abuse and medical service providers, application for benefits, advocacy, crisis intervention and other appropriate supports. Services are provided based on individual need and to support the recovery, health and well-being of the person to enhance their quality of life. Group activities are planned on a quarterly basis or more often if appropriate to provide persons living at the properties an opportunity to hear informative/educational speakers, interact socially with each other, and address issues with property manager and tenant case manager. The TCM encourages and supports involvement in the community. The persons served are in fact involved in many ways through participation in day programs, education, employment, volunteer opportunities, faith organization, advocacy opportunities, etc. Persons are monitored and supports enhanced when appropriate.
This service is provided to persons who are living in Western North Carolina Housing, Inc. properties in the Asheville, North Carolina area through a contract with Western Highlands Network. Persons receiving tenant case management are not billed for services and services are not billed by tenant. Synergy In Action, Inc. invoices Western Highlands Network for expenses incurred. Property Management is provided by Tri-State Management. The Tenant Case Manager coordinates closely with the property management staff.
These properties provide permanent supportive housing for adults, with mental illness or intellectual/developmental disabilities in HUD and NCHFA apartments or cottages. This service is distinctive in that it provides the supports needed to help the person obtain and maintain independent living. Level of support is based on individual need and choice of person served. The Tenant Case Manager works closely with the property manager to resolve issues which may lead to loss of housing.
The Tenant Case Manager coordinates closely with families, providers of mental health, developmental disabilities, substance abuse and medical services with signed release from person served. Information for available community resources is continually updated and utilized to connect persons served with need resources.
The Tenant Case Managers is available Monday thru Friday, 8:00am to 5:00pm. Support is available 24 hours a day, 7 days a week for emergencies. The TCS maintains an office at Nantahala Apts. Service may be provided in Tenant Case Managers office, in the home of the person or in the community.
